FHTH is a division of Future Hope Health Group.  In addition to our pediatric division, we also offer functional medicine and state-of-the-art neurofeedback through New Mind Kentucky!
Our goal at FHTH is to bring health and healing into adult primary care… just as we have been doing in pediatrics for over a decade.

About Us

The pathway to Future Hope Total Health has been an interesting journey.  For those of you who have been with us for a while… thank you for being a part of that journey!

In 2008, we opened Future Hope Pediatrics.  Our goal was to provide pediatric care with excellence (through a conventional medical model).  Over the next few years, our practice both grew and transitioned into a more holistic approach to care.  

While I was always a “minimalist” in care—understanding that children largely get well on their own—I began to understand the power of supporting those innate healing pathways.  My fundamental understanding of medicine shifted gradually, but dramatically, during that season.

In 2017 that led us to open Future Hope Total Health, a “functional medicine” practice geared towards treating chronic medical issues in both adults and children.  It turned out that my approach to pediatrics would serve me well in that arena.  As I trained, I began to understand that self-healing is fundamental to who we are as humans.  We are created to heal at the deepest levels of our being.  My role was to partner with our patients to find the underlying causes of disease, remove the irritants (toxins) blocking healing, and give the body the tools it needs to do the powerful work of healing. 

Over the last few years, we have repeatedly heard the same question from parents of our pediatric patients “where can I get this kind of care for myself?”  We heard similar questions from our Total Health patients.  

While 2020 was undoubtedly a year of transition and change, some of them were amazing.  It gave us an opportunity to re-evaluate how we could best meet the needs of our families and start to change the current paradigm of medicine.  With that in mind, we decided the time had come to create an adult version of what we have watched emerge on the pediatric side…  

bringing forward the ancient ideas of healing, forgotten over the last century, into the now.  

And, allowing health and healing in all its modalities to coexist. 

We are not your ordinary primary care practice.  We aren’t the right practice for everyone.  

But, if you are looking for medical care that focuses on the prevention of disease, and real health and healing, then we look forward to meeting you and helping you to achieve those goals!



Philosophy

Our goal at Future Hope is to be on the front edge of a new paradigm in medicine.  
Our current “healthcare” system is more concerned with disease management than prevention or resolution of disease.

Conventional allopathic medicine focuses on downstream problems.  Whether it be an infectious agent or a chronic problem, we know there are always underlying predisposing issues.  Conventional medicine tends to address the symptom without ever getting down to the root causes of the disease.  

At Future Hope, we strive to look for the “Why” and not just the “What”.  We want to prevent disease and when necessary treat the underlying issues and not just the surface-level symptoms.  This requires a partnership in which both the provider and patient are committed to action that often requires change.  This isn’t a “pill for every ill” philosophy.  It is real health care.

This won’t be your conventional medical practice.  We will strive to bring health and healing into adult primary care… just as we have been doing in pediatrics for over a decade.

We have high expectations for ourselves as we serve our patients.  In return, we do have expectations of our patients.  



What do we expect?

  • An atmosphere of mutual respect

  • Understanding that our staff and providers are also human, and grace should be extended to them

  • Our goal is to provide care that goes beyond conventional medicine. As a result, we may ask our patients to make lifestyle changes. If you have no desire to do so, we may not be able to optimize your care.

  • While we will do the best we can to quickly address your needs, there are some parameters

— Refill requests require 72 hours’ notice (Don’t count on your pharmacy to contact us.  They are busy too and your refill needs are not at the top of their priority list)  Please call us BEFORE you are out of your medication!

— No refills will be sent after hours or on weekends.  No new prescriptions will be sent out after hours

— Our providers will be available for emergency calls after hours.  Routine questions and non-emergency calls should be reserved for office hours.  Please consider whether your needs truly fit an emergency status.  

— During office hours you may reach our providers and leave a message by phone.  We will be considering other ways to communicate as we move forward.


If you are looking for the status quo, we probably aren’t the place for you.  But if you are looking for a different approach to health care that focuses on real health you will find a home here.




Basic Membership Information



Fee structure:

A one-time fee of $100 to be paid upon acceptance of your application and signing of the Membership Agreement  (Fee will be waived for current patients of Future Hope Pediatrics or Future Hope Total Health)

  • $65/ month for adults

  • $35/ month per child with an adult membership— but capped at two children per family.

  • Children without an adult membership— $45/ month— capped at four children per family.

How is it paid:

There are two options for paying your membership fees.  

  • You can pay for the entire year upfront and get a 10% discount.

  • You may also pay monthly with a credit/debit card kept on file through Hint Health. It is your responsibility to make sure that information is kept up to date.


What is included:

Details will be spelled out in your membership agreement, but basic benefits include:

  • Yearly well-checks for adults

  • Well checks as recommended for children

  • Acute care sick visits

  • Chronic disease management

  • Sports physicals

  • Basic in-office labs

  • Discounts on outside lab services

  • Direct primary care is a financial arrangement made directly between a patient and healthcare provider.  It cuts the insurance providers out of the process, erasing the need for patients and providers alike to file health insurance claims.” (https://www.goodrx.com/insurance/direct-primary-care)

    When you are accepted into our DPC-based practice, you will pay a set monthly fee that covers agreed-upon in-office services— including well checks, sick visits, in-office tests, procedures, etc.   

  • In the world of medicine, insurance companies create a separation between provider and patient.  This third party dictates the care a provider can provide and can limit what the patient needs.  When the third party’s business priority is net revenue, it can compromise what is best for the patient.

    Shrinking reimbursement from insurers drives providers to see more and more patients in order to survive.  That translates into rushed and shorter appointments and less face-to-face interaction.  That is a major reason that most providers are a part of large medical businesses and there are fewer and fewer stand-alone practices.  When business models dictate care, both the providers and patients suffer the consequences.

    Detailed diagnosis codes and access to medical records also impact the confidentiality of the physician-patient relationship.  We feel strongly that your medical decisions are yours and should be held in the strictest confidence.

  • We encourage our patients to carry a high deductible or major medical plan or a plan with a health savings account to provide coverage for catastrophic medical expenses or referrals to specialists as needed.

  • Maybe.  Contact your employer to find out if your particular plan offers that option.

  • Our goal will be to see you on the same day or the next.  

  • Our providers are available after hours for emergency questions.  You will never be referred to an answering service that has a different philosophy of care.

  • While there is nothing else like face-to-face care, we do understand the value of telemedicine visits.  However, we are unlikely to prescribe medications unless you have been physically seen and examined in the office.  Yearly well checks will also need to be done in person.

  • We will refer you to specialists as needed.  (If you carry insurance we may or may not be able to meet their referral requirements).  However, with the additional time we are able to spend with you, we may be able to minimize your need to go elsewhere.  Our basic philosophy is one of prevention and holistic treatment… which is very different than what most other providers offer. 

  • There are two options for paying your membership fees.  You can pay for the entire year upfront and get a 10% discount.  You may also pay monthly with a credit/debit card kept on file through Hint Health.  It is your responsibility to make sure that information is kept up to date.

  • If you move away from the area we can continue to provide care via phone or telemedicine.  Once you transition care to another provider (i.e. we transfer records) we will cancel your monthly obligation.

  • All of our providers are certified in adult and pediatric primary care.  

    Dr. Laura White is the medical director and oversees the vision, philosophy, care plans, and supervision of all visits for Future Hope Health Group.

  • Our membership agreement is designed to clearly lay out what is (and is not) included in our services. It is a one-year agreement that can be canceled by either party as per the agreement.

  • Our membership agreement is designed to clarify expectations and provide financial safeguards for both our patients and our practice.

    While we strive to provide the best medical care and services for our patients, we understand that there may be times when a membership needs to be canceled.

    If that need arises, we will allow you to terminate your membership as per the terms of the membership agreement.

  • Phone and telemedicine options are available

  • If you are hospitalized, your care will be directed by a specialist or a hospitalist (a dedicated in-patient physician who works exclusively in a hospital). However, they will work closely with hospital specialists to provide input if indicated, stay abreast of our situation and provide follow-up care.

  • No, unfortunately, in Kentucky it is currently illegal for providers to privately contract with Medicaid recipients

  • Yes, you may join the DPC if you have medicare. However, we are not Medicare providers and will not bill or accept payment from medicare.

  • Yes, please contact our office for options.

  • While we think our membership opportunity is a great option, there are times when a single visit option may be warranted.

    Single visits are available with our providers at a flat-rate fee of $250.

    A single visit does not establish us as your primary care provider and does not include follow-up care.
